SUPPORT TO PEOPLE LIVING AT HOME WHO HAVE DEMENTIA

We have lots of experience of supporting people living at home who have some form of Dementia. We know that there are many different types of Dementia and often symptoms can go undiagnosed, leading to frustration for the Individual and their family. We have a specialist team of staff who are specially trained to level 2 of the Qualification Credit Framework. They have experience in supporting people with Dementia and can work with you and other professionals to recognise the signs and symptoms, to support an early diagnosis, to gain access to treatment and further support from the specialist agencies. Additionally, we can support family carers in the care and support of the Individual with Dementia. We can monitor their safety,  when it becomes unsafe for the Individual to remain living at home alone.

 
EXAMPLES OF THE SUPPORT WE CAN OFFER

The level of support will be planned with the Individual, their family members and any other professionals involved. Our aim will be to provide a service that:

  • Maintains a simple routine in an environment the Individual is familiar with.

  • Use notes and pictorial reminders.

  • Make recommendations for Telecare Equipment, to remind and promote the safety of the Individual.

  • Use personal possessions around the home, to make conversation and comfort, gaining and using information of people, events and matters of importance to the Individual, from both past and current day.

  • Prompt the Individual with their loss of memory.

  • Monitor sleep and behaviour patterns, reporting noted changes and assessed risks to family members and involved professionals.

  • Liaise with family members and other professionals to ensure the Individual is safeguarded.

  • Support the Individual with their personal care with their consent.

  • Offer support and respite to family carers.

 
LONGER TERM CARE

Initially our services may be for frequent short visits, which may increase in duration around the increased needs of the Individual and their family carers. In time it may be that it becomes unsafe for the Individual to remain living at home. We can offer support to the family and the Individual at this time and with permission share our knowledge of the Individual to any residential or nursing home, to ensure any move there is smooth and they are well informed about the Individual.

Our care staff are trained to support people with Dementia and we can help ease the worry you or your family may have. We will work with you to ensure that your loved one is well looked after and that you are informed about everything that we do. We will make suggestions when we feel it appropriate to the care and support we provide.
